Bipolar disorder is a common, severe and recurrent illness that is associated with high rates of morbidity, disability, and mortality. Mixed states (MS) is a presentation of bipolar illness that involves concurrent symptoms of mood elevation and depression/dysphoria. MS is frequently associated with psychosis and co-morbid substance use disorders and poor prognosis. Treatment options for MS are limited, particularly for the depressive domains of the episode. Additionally, there is a paucity of data on the acute and long-term safety and efficacy of combination treatment in MS.

This research is a 20-week open-label study of Iloperidone added to ongoing treatment regimen with mood stabilizers in the acute maintenance of mixed state. This study will assess the acute and long term bimodal efficacy of iloperidone (IL) as an adjunct treatment for patients with a MS in bipolar disorder. Primary efficacy will be measured by change from baseline on both the manic and depressive scores using structured rating scales.
